Support worker

The position is responsible for the provision of appropriate care, support and advocacy, client programs and activities, and provision of case planning and case management tasks as directed by the program manager. All services will be delivered in line with the programs Duty of Care. Performance of all duties required to operate the service must align with the established protocols of the organisation and the Service Agreement.

Each day will be distinct however your key areas of responsibility will be:

  • Developing and maintaining effective relationships with clients, nurturing, and supporting clients while assisting in the development of their skills needed to transition to alternate accommodation
  •  Assisting with the intake and assessments of clients
  • Working in accordance with Individual Case Plans and encouraging client participation and engagement in care plan activities
  • Contributing to effective client household management practices
  • Providing information, support and advocacy for clients accessing the service
  • Maintaining communication networks with other appropriate agencies
  • Ensuring secure management of all program files and compliance with applicable privacy legislation and policies
  • Performing work in accordance with organisational policy, process control plans and work instructions 

 

About you

  • Proven ability to work autonomously and display initiative, self-motivation, time management and solution focused skills
  • Proven ability to demonstrate strong capabilities around buildiing relationships with and motivating vulnerable young people, assessing needs and advocating for the client
  • Proven ability to work with a team in ways that are respectful, transparent and acknowledging of an individual's skills, abilities and capacities
  • Ability to comply with all legal, ethical, confidentiality and regulatory requirements

Required Qualifications

  • Cert IV in Child Youth & Family Intervention, Community Services or equivalent
  • Previous experience in a similar role

 Required Licenses and Checks

 

 

·         Ability to pass a National Police Check – no Serious/Criminal/Court Record

 

·         Current Blue Card (working with children and young people)

 

·         Current C Class Drivers Licence (Qld)

 

Practical Requirements

·         Work out of normal hours of duty may be required

 

·         Travel across region may be required

 

·         Ability to perform the physical requirements of the role in a safe manner
